Important NOTICE . . . As of 7/01/08 you MUST have your Signiture on file for Quick Charge with Human Resources at your Hospital to receive Payroll Deduction orders. If you signiture is not on file, your order will not ship.
Please check if your signiture is on file for payroll deduction. If you place an order with us and accounting does not have your signiture, we will send you an email notification that you have 1 week to get your signiture on file and contact us that you have signed up. A simple email reply from your order confirmation will do. If you do not sign-up within the week, your order will be cancelled.
Once your signiture is on file, all orders placed in the future will ship immediatly. If you do not want to sign up right now, simply use a credit card for your purchase. |
